Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

My mother was there for over nine years, and I never once even thought about changing nursing homes. They made sure I had everything I needed to care for my mom and anything I requested was provided rapidly. The administrator in this nursing home is a caring individual together with her nurses and DON. If I had to do it again, that is the place my mother would still be. I still often go into the nursing home because I made friends there. I cannot put my gratitude into words.

[review] => Before my mother in law passed a few weeks ago the last time we spoke with her was 2 weeks prior. The facility would put us on hold and no one would answer. My husband called his mom everyday as long as we have been married 13 years and this truly broke us.
She was in Rehab, rehabilitation!!!!!!!!
Neglect, Pure neglect is what I know about this place. The last call we made was Wednesday February 5th and I asked a nurse to take a walk down the hall and check in on her because something wasn’t right. She stated “she is a little weak because she has pneumonia “. And by the next night she was rushed to the hospital and died in transit!
Few questions
1. Why did it take so long for anyone to realize her horrible condition
2. Who was the nurse or CNA on duty the past two weeks
3. Why don’t the phones ring on the opposite end of the phone
4. Why couldn’t I speak with the head administration
5. How did her roommate die also
6. What’s the death rate here
I requested medical records and was told only a Power of Attorney can request that. She’s dead! She left in her right state of mind.

They had a special section for short-term rehab, for long-term care, and another section for dementia or Alzheimer's patients. But everything was very clean, and they had separate areas for each group.

[review] => The dining experience for your loved one at this facility is like going to an upscale restaurant.
The employees take the resident orders and serve them just like a restaurant.
Also, the facility is extremely clean and the employees seem happy and engaged with the residents they serve.. There’s a lot of laughter and joy here.
The administrator has put a lot of effort into revitalizing this facility and if you haven’t been here lately, you should come. The leadership is evident here.
It’s great to be able to write a five star review for a nursing home.

**Concerns Regarding Facility Conditions and Staffing**
**Facility Environment:**
The overall condition of the facility is alarming. The food provided is often inadequate and does not match the menu descriptions. Numerous health and hygiene issues are present, including pests such as roaches, spiders, rats, and bed bugs, as well as mold in many areas. The unpleasant odors in the hallways are overwhelming, and litter, including cigarette butts, is prevalent. Furthermore, due to neglect, some residents have developed tunneling wounds, and others have suffered from maggot infestations in untreated wounds.
**Admission and Staffing:**
The quality of staffing and admission processes appears to be inconsistent, with a roughly equal mix of competent and subpar personnel. Those who do not engage in favoritism or gossip often find themselves at a disadvantage. Concerns about abuse of power may lead to retaliation, where individuals face repercussions for reporting misconduct. When state inspections occur, staff members without the appropriate CNA licenses are sent home, which highlights the importance of proper licensing. Unfortunately, reports of mistreatment towards residents often go unaddressed, allowing abusive staff to remain employed.
Additionally, staffing issues result in inadequate care for residents. When staff members are required to fill in as CNAs, they frequently neglect their responsibilities, leading to residents being malnourished and dehydrated. Staffing management often manipulates schedules, leaving employees without proper notice and subjecting them to unfair treatment, all while offering insufficient compensation for the demanding work expected of them.
**Nursing Staff:**
The nursing staff also reflects a mixed quality of care. While some nurses demonstrate a genuine commitment to their roles, others appear to prioritize their salaries over patient care. Instances of medication mishandling and even narcotics theft have been reported, yet these issues are often swept under the rug to protect licenses rather than addressed appropriately. Additionally, nurses often face challenges in obtaining necessary supplies for residents, further compromising their ability to provide adequate care.
**Certified Nursing Assistants (CNAs):**
The performance of CNAs is concerning, with a significant number failing to meet the required standards of care. Many CNAs are difficult to locate when assistance is needed, and some may avoid responsibilities altogether. This neglect has led to serious consequences, including increased risks of aspiration and choking among residents. The expectation for CNAs to provide nightly showers in an understaffed environment adds to the unrealistic demands placed on them.
**Conclusion:**
The overall environment of this facility raises serious concerns about the quality of care provided to residents. Despite any positive reviews that may exist, it is crucial to recognize that many of these may be influenced by pressure from staff and administration. It is advisable for families to seek alternative facilities that genuinely prioritize the well-being of their loved ones, as the superficial improvements may not mask the underlying issues present within this establishment.

[review] => My family member was admitted for a second time at this facility. The first time, everyone did their jobs (administration, nurses, PT, etc.) and things were smooth. The 2nd admission was a total 180. I had to ask for a meeting w/admin to communicate the end goal/care plan. I got the meeting 2 days prior to them discharging her. She was there for over a month. Nurses were short staffed. A CNA had to serve food trays on one occasion, their clients had to wait to be changed and tendered too. Admin only concentrated on money and what they could get i.e. house and client's income. They say to let them know about concerns, which I did on several occasions and I got slow responses or no responses at all. Messages were taken but seldom returned. They did us a favor when my family member was discharged. On a side note, the assigned nursing and physical therapy staff was pretty good. They were short handed, but they tended to patients as quick as they could. My family member will NEVER be sent there again. If I could give a negative numerical number I would. I can only pray that those who are there will obtain better services and care.

There were more patients than people to take care of them. [review] => I am extremely pleased with Jacksonville's nursing and rehab facility. This is my second time being here from my second hip surgery for rehab. I saw new and familiar faces and was welcomed with open arms. The Facility Director, Raelyn Covington, was the first to greet me. The CNAs, Zykerra, Axel, Judith, Ms Johnson, Darlene, and ALL the other ladies who helped me were terrific as usual. Daryl in OT and Andrea, Michael, and staff worked me where I'd be ready for release. Mrs. Gail and Denzel, I really had a ball. The nurses, Mrs Shanika, Mrs Pat, and Ms Sandra made getting stuck four times a day an absolute pleasure. The Cafe workers did an excellent job. Words cannot express my gratitude. The staff is top rate. Now the food needs work, but on a scale from 1-10, I give it a 5. I met the person in charge of the food, and working on making improvements. The entire staff are caring and personable. The physical therapy staff are top notched. I went from a wheelchair to a walker and then a cane. The wound care dept treated me with dignity and respect. The first day that I left, my mother I cried, I was sad not because anyone had done or said anything to me or her. It was just the idea of her being there.From Day one when I open the door, I was greeted with a smile, and everyone kept smiling and was so sweet and nice to me and my mom, I spoke with Raelyn J Covington the administrator about a few minor concerns that I had, which were taken care of immediately, I had so much peace about my mom being there coupled with the fact that she was actually having fun with all the activities they have for there patients. On discharge we could hardly make it to the front door without her, being hugged and them, letting her know how much she would be missed, The entire staff at Jacksonville nursing and rehabilitation center was awesome but I would like to send a special thank you to Kamisha Coleman this young lady went above and beyond she paid attention to the little things that ppl may not think mean much,but they mean much .

[review] => My grandmother was in this facility last year and I think enough time has passed that I can now share my opinion of this place. Unless there has been a complete overhaul do not send your love ones here. Roaches are everywhere, there are not showers in the rooms so everyone has a shower schedule to wash only 3 days a week. The nurses ignore patients for hours if they use the call light and are very rude to the sick and elderly. They left my grandmother in her urine spackled clothes for hours. She was not given PT for days. It took me a week to get her out of there and in her new place she was much better taken care of.
Taking care of the sick and elderly is tough, but this place is absolutely disgusting for their treatment.
Also the facilities are very old and outdated. They need to redone the entire place.

Comparing all the places I've looked at, it's nice - landscape, driveway, cleanliness, parking lot, and the room she's in. She has a nice room. She's in a room by herself. The staff for the short-term rehab is is good. They had a nice physical therapy team. They did very well with her. They helped her tremendously. The food is medium, not great, not terrible. It was recommended by another place, so that's how we ended up going and checking it out, and it looked as good as our other options. They could improve on the CNAs, their attitudes, cleanliness, making sure the trash cans are dumped, and responding in a more timely manner with any request or what they're supposed to do. It seems like when you ask for something it takes an hour for you to get it.

I followed my companies instructions and took a photo of the food on a bench by the door (not on the ground and not in front of the doors where it would have impeded the ability to leave the facility.) Upon setting the order down, an angry looking white woman with blonde Ramen looking hair came out and made a snide comment "oh you're just going to leave the order outside on the bench? Okay." I responded to her as she was flipping her hair and walking away to the parking lot that I was following the instructions on the order and also that the door says "for the safety of our guests the doors are locked 24/7". She rolled her eyes at me and rudely stated that they don't lock the door (which is true, I watched her walk back in without a key or code.) Imagine how unsafe this environment is that anyone can walk in or out. Regardless I followed my company's procedure.
As I was walking back to my car, Nancy (I'm assuming as she grabbed the food) angrily said "I guess I have to be more specific with my instructions." Along with a derogatory comment to me. I responded with "Yes, you do. If you want the order handed to you, you need to choose the option that the app gives you to hand to customer. " she was then further derogatory to me and made me feel very unsafe and continued to harass me as I left.
I feel terrible for your patients, you have no respect for someone doing a service for you, I can only imagine how you treat those that you are supposed to be helping. Do better or get out of the medical field. I'm disgusted.

They gave me rehab with a fracture. I had a fracture and they said, "No, he's alright." I didn't see them treating anybody halfway decent. They just did not care. I think they had one good CNA, and that was it. I'd give them a five on cleanliness. They thought I had just a bruised hip, but it was a fractured hip. I don't know if they took another x-ray. It hurt bad, so I made them leave me alone for a couple of days. On the third day, they came in and said, "We got to get you out of bed and walking," so I got out with a walker and walked around. The therapist people were pretty good. I have to give them credit. They had one activity in the afternoon, and that was bingo. I went to one bingo, and they put a thing around my ankle; they thought I would run away. This place was not right. Do not send anybody there.

Also, I asked the nurse if they gave him his medication, she stated oh, It could of been the hours of 6-10 am but never a definitive time, they never gave him his medication because his pressure was through the roof. The pharmacist said that rehabilitation place don't know what they are doing. This place is a disappointment. I spoke to his insurance company and reported them. This place is the worst place anyone can go to. Please stay away! I'm begging anyone to listen , please stay away. The two people who seem that they were concerned was Ms. April and Ms. Sade. The nurse Leandra was nasty and mean. I should of transferred when I had a chance but the therapist did a good job. This place need to be reported and shut down. Also, this place have a serious smell coming through the vents. My next step is to contact joint commission and report this place. Its not even fit for an animal and I feel horrible that I had my dad stay there.

They would only tell that poor soul they would be back "Later", and would never come. My father was left unattended with soiled "Depends". When I came for him the fourth day, the Staff harassed me about taking him home! Finally, while I got my car, they got him dressed, but with no protective underclothes. Of course, they were soiled by the time we arrived home. Beware of leaving a loved one at this facility.

I visited twice a day while my grandmother was there and never had any issues with cleanliness. Staff was always in the halls mopping or buffing the floors. We did have one mechanical issue, her bed was not working properly however maintenance visited the same day and was able to fix the problem in no time. The main reason of her stay was rehabilitation, and the rehab department did just that! They work diligently to ensure my grandmother could preform daily routines once she went home. I would highly recommend them to anyone.
On a side note... the food is the same as most hospital and facilities, however I do give them props for being creative on flavors and not having the same thing twice during her stay!
Definitely consider if needed!

The first week my mom was there she couldn’t move so she had to eat in her room in the bed, the bedding wasn’t changed so she was lying in crumbs. Her feet were dirty because she hadn’t had a shower.
I was visiting my mom and was there for 2.5 hours and my moms room mate whose confined sat in a chair for the time I was there, no one checked on her for an hour and when they did they told her they were coming back to put her in her bed. An hour later before I left she was still in the same position and no one came.
The weekend staff is lazy and rude and make a lot of noise with no regard for the residence there. I’ve made 3 complaints now further action needs to be taken.

[review] => This place likes to miss treat there patients and leave them in there own piss and crap for hours and you ring the button and it takes them hours to get to you then they leave there patients in chairs or wheel chairs for them to fall out of it the staff is to busy laughing and talking to even worrie about there patients that need help ive seen some of there patients sleeping on the floor as soon as you walk threw the front door it smell straight like crap and piss it hits you bad its like the staff gives two craps about there patients and its truly sorry that there is a place like this that treats peoples family member's like crap

[review] => JNRC is a great facility. It does work hard to provide a caring place to rehab or receive care people can't get at home. Despite a few disgruntled employees who may have been let go for not following the rules, it is a good facility.

[review] => Right now, my mom is in Jacksonville Nursing and Rehab. The staff is great. They do a good job taking care of their patients. They really put their hearts into it. They have therapy for the seniors. There is an area where people can socialize. The rooms they have there are great. Their dining room is also nice. My mom has three meals a day.

I am a patient at Jacksonville nursing and rehab and I have had an excellent stay. I would like to give a shoutout to Shania Lawrence the best CNA and nasa for excellent care. The entire rehab team has been great, Darryl, jansen, and Michael p have went above and beyond for my rehab. Raelyn the administrator has also been very attentive to all my needs!
