Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => Let me start off by saying that anyone who has their loved one in a nursing or rehabilitation center,you are automatically nervous. The first day that I left, my mother I cried, I was sad not because anyone had done or said anything to me or her. It was just the idea of her being there.From Day one when I open the door, I was greeted with a smile, and everyone kept smiling and was so sweet and nice to me and my mom, I spoke with Raelyn J Covington the administrator about a few minor concerns that I had, which were taken care of immediately, I had so much peace about my mom being there coupled with the fact that she was actually having fun with all the activities they have for there patients. On discharge we could hardly make it to the front door without her, being hugged and them, letting her know how much she would be missed, The entire staff at Jacksonville nursing and rehabilitation center was awesome but I would like to send a special thank you to Kamisha Coleman this young lady went above and beyond she paid attention to the little things that ppl may not think mean much,but they mean much .

[review] => My grandmother was in this facility last year and I think enough time has passed that I can now share my opinion of this place. Unless there has been a complete overhaul do not send your love ones here. Roaches are everywhere, there are not showers in the rooms so everyone has a shower schedule to wash only 3 days a week. The nurses ignore patients for hours if they use the call light and are very rude to the sick and elderly. They left my grandmother in her urine spackled clothes for hours. She was not given PT for days. It took me a week to get her out of there and in her new place she was much better taken care of.
Taking care of the sick and elderly is tough, but this place is absolutely disgusting for their treatment.
Also the facilities are very old and outdated. They need to redone the entire place.

[review] => I am not surprised at all that this facility is under investigation for neglect and abuse.
I was contacted by a third party to deliver food here for Nancy.
The instructions said to place the order at the door. I followed my companies instructions and took a photo of the food on a bench by the door (not on the ground and not in front of the doors where it would have impeded the ability to leave the facility.) Upon setting the order down, an angry looking white woman with blonde Ramen looking hair came out and made a snide comment "oh you're just going to leave the order outside on the bench? Okay." I responded to her as she was flipping her hair and walking away to the parking lot that I was following the instructions on the order and also that the door says "for the safety of our guests the doors are locked 24/7". She rolled her eyes at me and rudely stated that they don't lock the door (which is true, I watched her walk back in without a key or code.) Imagine how unsafe this environment is that anyone can walk in or out. Regardless I followed my company's procedure.
As I was walking back to my car, Nancy (I'm assuming as she grabbed the food) angrily said "I guess I have to be more specific with my instructions." Along with a derogatory comment to me. I responded with "Yes, you do. If you want the order handed to you, you need to choose the option that the app gives you to hand to customer. " she was then further derogatory to me and made me feel very unsafe and continued to harass me as I left.
I feel terrible for your patients, you have no respect for someone doing a service for you, I can only imagine how you treat those that you are supposed to be helping. Do better or get out of the medical field. I'm disgusted.

I just watched a news story on Action News where an employee working for their cleaning service reported this facility for excessively roaches and filth. Complete with video footage. Disgusting having roaches in beds and rooms. And I believe it even w/out the video. My aunt before her death was here in 2014 and it was deplorable. Her daughter and my dad had her removed and relocated to another facility until her demise.
The State needs to investigate and hold this facility accountable..

Please don’t let your loved ones reside here. There are roaches and the facility lack proper cleanliness. [review] => My dad a stroke patient was just discharged and the nurses never sent my dad any of his medication to his pharmacy, they also never gave him his
Prescribed medication before he left, his pressure was high before he left and I believe they deliberately sent my dad home to fail. I had to call the rehabilitation over 10 times to send my dad's medication to CVS and when I got there ,they said they just sent it over 20 minutes ago. Also, I asked the nurse if they gave him his medication, she stated oh, It could of been the hours of 6-10 am but never a definitive time, they never gave him his medication because his pressure was through the roof. The pharmacist said that rehabilitation place don't know what they are doing. This place is a disappointment. I spoke to his insurance company and reported them. This place is the worst place anyone can go to. Please stay away! I'm begging anyone to listen , please stay away. The two people who seem that they were concerned was Ms. April and Ms. Sade. The nurse Leandra was nasty and mean. I should of transferred when I had a chance but the therapist did a good job. This place need to be reported and shut down. Also, this place have a serious smell coming through the vents. My next step is to contact joint commission and report this place. Its not even fit for an animal and I feel horrible that I had my dad stay there.

[review] => If I could submit with 0 stars I would. My father came here via Hospice, scheduled for a 3 night / 4 day stay, while family handled his household affairs out of town. Even the lobby smelled of urine. The first night, he called for assistance to use the bathroom (he was a fall risk). 20 minutes later no one showed up to help him. When I went to inquire on assistance, I was told the employee assigned to his area was on dinner break and would not return for another 30 minutes! The first morning, they forgot his breakfast. The second day they forgot his dinner, and argued about it! His roommate was non verbal, but tried obviously to communicate for assistance. They would only tell that poor soul they would be back "Later", and would never come. My father was left unattended with soiled "Depends". When I came for him the fourth day, the Staff harassed me about taking him home! Finally, while I got my car, they got him dressed, but with no protective underclothes. Of course, they were soiled by the time we arrived home. Beware of leaving a loved one at this facility.

[review] => This 5 star shoutout mainly reflects the therapy department! The care they gave my Mom was the best. After dealing with total incompetence at previous facilities in GA where she was for 3 months and told she’d probably never eat regular foods again as well as only thickened liquids because of her stroke……..it was definitely worth it to get her back to Florida at this facility. Her therapy team was caring, loving and willing to do whatever it took to meet our goals. We cannot express how thankful we are for all they did. Mom is doing well and can eat a steak! .

[review] => Great experience. I heard bad things about this place from a friend. Boy was he wrong. I was in another facility then transferred here. This staff is so caring. I didn't meet anyone at this facility that wasnt nice and caring. They always took my vitals, made sure I had water. Everyone did their job. I was never ignored. They always took my blood pressure twice to make sure it was accurate came and checked on me all through the night. Very caring staff. Gonna miss this place. The cnas Ms Ruby and Ms Helen was the best.. they went out their way to help anyone. Great staff These people cared. Food was never enough wonder who they thought they were feeding 8 year olds. Staff is great. If I had to go there again I would.

Unfortunately she moved to a nursing facility on Max Leggett parkway where she passed away in just a few months. I wish she had stayed here where she received the proper medical care . [review] => Jacksonville Nursing & Rehab, took very good care of my loved one. Not only was the nursing staff always kind and courteous, they showed respect and were highly skilled. I visited twice a day while my grandmother was there and never had any issues with cleanliness. Staff was always in the halls mopping or buffing the floors. We did have one mechanical issue, her bed was not working properly however maintenance visited the same day and was able to fix the problem in no time. The main reason of her stay was rehabilitation, and the rehab department did just that! They work diligently to ensure my grandmother could preform daily routines once she went home. I would highly recommend them to anyone.
On a side note... the food is the same as most hospital and facilities, however I do give them props for being creative on flavors and not having the same thing twice during her stay!
Definitely consider if needed!

[review] => My mother is there for therapy and rehab. I was there to visit her and there were roaches crawling around, I was told there is a roach problem at the facility. The first week my mom was there she couldn’t move so she had to eat in her room in the bed, the bedding wasn’t changed so she was lying in crumbs. Her feet were dirty because she hadn’t had a shower.
I was visiting my mom and was there for 2.5 hours and my moms room mate whose confined sat in a chair for the time I was there, no one checked on her for an hour and when they did they told her they were coming back to put her in her bed. An hour later before I left she was still in the same position and no one came.
The weekend staff is lazy and rude and make a lot of noise with no regard for the residence there. I’ve made 3 complaints now further action needs to be taken.

Wonderful place, the employees are very friendly and welcoming. New administrator, Raelyn Covington is great. Wonderful energy, positive attitude and always greets you with a smile. Tammy, DON and all unit managers are wonderful as well. Keep up the great work!!!
